I accept direct private payment and payment by several major medical health insurance plans and EAP plans.
I am a network provider for the following major health insurance plans
· Anthem / Blue Cross
· Optum
· United Behavioral Health
· Aetna
I will need information from your health plan identification (ID card) before your first session so I can confirm co-pay and coverage. At the time that we schedule your first session I will give you instructions on how to send that information to me before your session.
If you have any other health insurance plan for which I am not a network provider, I can provide you with a paid bill showing you paid my customary Private Pay fee (see below), plus diagnosis code and all other information needed for you to apply to your insurance company for Out-Of-Network reimbursement. Check your policy as the Out-of-Network coverage varies from company to company.
I am a contracted provider for the following employer-funded Employee Assistance Programs (EAP) :
· Anthem
· Aetna
I do not charge any fee to you for my services under your employer's EAP program. There is, however, a limit to the number of sessions provided - usually between 4 and 8 - depending on your employer's program. After you have used the available number of EAP sessions your normal health insurance coverage still applies.
My fee for direct private payment is $150 for a standard 50-minute session.
Private Pay fee is negotiable, in cases of financial hardship.
I accept payment by cash, check or credit card (Visa or MasterCard) at the time of service.
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance, or who are not using insurance, an estimate of the expected charges for medical services, including psychotherapy service. You can ask me or any other health care provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
